Discharge Services
Going home from the hospital after an illness or surgery can be stressful for patients and family members. Our goal is to make your transition as smooth as possible.
Before your doctor discharges you from the hospital, a multi-disciplinary medical team meets to develop your discharge plan. This plan takes into account your medical and social needs. The team includes a nurse, a social worker, and representatives from Dietary Services, Respiratory, Behavioral Health, Physical Therapy, and Pharmacy as required. Our goal is to provide a plan that supports your ongoing recovery.
Your discharge plan may include:
- Referrals to resources like home health or hospice
- Infusion services
- Durable medical equipment like a walker or wheelchair
- Prescriptions for medications
We work with patients and family members to provide emotional support during the time at the hospital. We also share information concerning community programs that provide special ongoing support for the patient and family. Often this support extends beyond the walls of Newberry Hospital and involves other helpful community resources.
