Medical Records
Newberry Hospital’s Health Information Management Department fulfills requests for medical records for all inpatient and outpatient services. We are happy to assist you in obtaining copies of your medical records upon receipt of the necessary release of information documentation. All medical information is confidential and protected by law.
Information from a medical record can only be released by authorization of the patient or legal health care representative. Newberry Hospital is required by law to keep your medical record for a minimum of 10 years after the age of 18; otherwise, records are maintained for a minimum of 10 years after the date of service.
Due to the cost of maintaining, retrieving and copying medical records, there will be a charge for copies, unless they are being sent directly to a healthcare provider such as a doctor or another hospital.
To request a copy of your medical record, you can download a copy of the “Authorization to Release/Obtain Patient Identifiable Health Information” form by clicking the button below. Once you print file, bring the fully completed form with you to the Health Information Management Department. A photo ID is required to verify your identity prior to the release of medical information.

Because we are committed to serving all members of our community and making each hospital visit as pleasant as possible, Newberry Hospital offers Spanish translation services to our patients and families. Spanish-speaking translators are available to help patients, family members or advocates with:
- The admissions process
- Meetings with doctors
- Talking with nurses and other clinical staff
- Visits to the ER
- Discharge
- Hospital bills
